Found this rock in small creek in Central Ky.
Is this a fish fossil?

Could be a section of a Cephalopod of some sort, something like Orthoceras:
[fossil-orthoceras-D3EWGW]

It depends on how old the rock is

Check the geological map of the area, I found this: https://ngmdb.usgs.gov/Prodesc/proddesc_16355.htm
Orthoceras were Ordovician, you need to check whether there are Ordovician outcrops in the area where you found the fossil.
